Are you (the applicant) self-employed or a business owner?

Date of the existing order (if any), next court date, when arrears began, or any deadline already running. (Used to assess if the matter is too urgent for the usual 3-business-day turnaround.)

Existing maintenance order (if any), a list of the child’s monthly expenses, proof of income for both parents, banking records, and any forms (e.g. J101) already filed. Helpful uploads include school invoices, medical costs, daycare costs, a summary expense sheet, and bank statements. Please avoid uploading large batches of individual receipts—prefer a consolidated summary where possible.
